WHO WE SERVE
Specialized Quality Care provides individualized supportive services to individuals with Developmental Disabilities. We equip individuals with the tools to navigate through their day to day lives. Specialized Quality Care focuses on empowerment and encouragement through building positive relations and cognitive reframing. Community integration and employment is a major part of our core values and beliefs. With the combined effort of family friends and team members we provide collaborative supportive services to our participants. Our team helps your loved one strengthen their ABILITIES by fostering their environment through person centered principles. Our service areas provide full care management, service delivery, and quality care for every individual served.
REQUIREMENTS
Individuals approved to receive funding through the New Jersey Department of Human Services Division of Developmental Disabilities (DDD). An individual must be 18 or older to be evaluated by DDD for functional eligibility for developmental disability services. An individual must be 21 or older to access DDD services.

COMMUNITY BASED SUPPORTIVE SERVICES & BEHAVIORAL SUPPORTS (EMBEDDED)
Community Based Supports Services (CBS): CBS services provide direct support and assistance for participants, with or without the caregiver present, in or out of the participant's residence, to achieve and/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community, as outlined in his/her Service Plan. Community Based Supports are delivered one-on-one with a participant and may include but are not limited to: assistance with community-based activities and assistance to, as well as training and supervision of, individuals as they learn and perform the various tasks that are included in basic self-care, social skills, and activities of daily living.
Individual Supports: Individual Supports are services that provide direct support and assistance for participants, with or without the caregiver present, in or out of the participant's residence, to achieve and/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community, as outlined in his/her Service Plan. Individual Supports may include but are not limited to: assistance with community-based activities and assistance to, as well as training and supervision of, individuals as they learn and perform the various tasks that are included in basic self-care, social skills, and activities of daily living.
In-Home Respite: Services provided to participants unable to care for themselves that are furnished on a short-term basis because of the absence or need for relief of those persons who normally provide care for the participant. Respite may be provided in the participant’s home, a DHS licensed group home, or another community-based setting approved by DHS. Some settings, such as a hotel, may be approved by the State for use when options using other settings have been exhausted.
Behavioral Supports (Embedded) NEW: Behavioral supports typically involve strategies, interventions, and services aimed at addressing challenging behaviors exhibited by individuals with developmental disabilities. These supports may include behavioral assessments, individualized behavior plans, counseling, therapy, and training for caregivers and support staff on managing challenging behaviors effectively while promoting the individual's well-being and quality of life.
